Clozapine therapy

Clozapine is an antipsychotic effective in treatment-resistant schizophrenia. It is devoid of numerous adverse effects of several other antipsychotics, viz extrapyramidal symptoms, tardive dyskinesia or elevation of serum prolactin. Severe adverse effects of clozapine are granulocytopenia or agranulocytosis. It is also useful in parkinson's disease. Constipation is a common adverse effect and may lead to paralytic ileus. Several drugs, smoking and consumption of coffee cause significant changes in clozapine concentrations. If clozapine therapy has to be discontinued for haematological reasons, it should not be reconsidered for the same patient.
