ESR is a simple method for evaluating acute phase reactants. The significance of a slightly elevated ESR is difficult to assess, particularly in the elderly and in menstruating women. ESR helps to differentiate inflammatory vs non-inflammatory joint disease (Rheumatoid arthritis versus Osteoarthritis). Changes in RBC morphology as anisocytosis, poikilocytosis or polycythaemia etc affect the ESR and can give normal values even in the presence of an acute phase reaction.
