Left Ventricular Hypertrophy

LVH may result from hypertension, aortic valve disease (Aortic stenosis or Aortic regurgitation) or mitral valve regurgitation. LVH is an independent and high risk factor for coronary disease. LVH, verified by ECG, is associated with a 6-8 fold increase in the risk of coronary disease or sudden death. A decline in ST-T change ("strain") doubles the risk. LVH can and should be prevented with effective therapy of hypertension. Echocardiography is a more sensitive investigation than ECG. Regression of hypertrophic changes indicates that blood pressure is well controlled, which improves the prognosis of the disease. According to recent studies, ACE inhibitors and ARBs are proving more effective than calcium-blockers, beta-blockers or diuretics, particularly in patients with several risk factors.
