Transient synovitis of the hip
Transient synovitis of the hip is diagnosed on the basis of typical symptoms and by exclusion of leukaemia, septic arthritis and Perthes´ disease. The disease is the most common cause of acute limp in a child aged below 10 years. The child limps or refuses to walk and complains of pain in the hip or knee, and keeps the hip in position of flexion and external rotation. Painful and limited internal rotation of the hip is a typical clinical finding. The ESR may be slightly elevated. An effusion of the hip joint is easily detected by ultrasonography. Treatment involves rest with the hip flexed. Joint aspiration in a hospital is necessary only if septic arthritis is suspected or the pain is severe. The prognosis is good.
